At this time I do not work directly with any insurance companies or providers.  Most of my patients are covered by insurance, but each policy is different.  I will be glad to work with you to provide any information necessary for your insurance.  Most simply need a cover letter from me along with a statement.  After you send in this information, they will reimburse you directly.
